Your medical records will remain under the direction of Dr. Eddie Ariss.
Florida law provides you with the right to request a copy of your medical records or have them transferred to another physician of your choice. Medical records must be retained for at least five (5) years from your last visit.

The Florida Board of Medicine recognizes that patient access to medical records is essential to ensure continuity of care. A duplication fee may apply in accordance with Florida law and will not exceed:
$1.00 per page for the first 25 pages
$0.25 per page for each additional page
